•  Application letter addressed to the Regional Director, and endorsed by the Municipal Fire Marshal and the Provincial Fire Marshal respectively 

 

•  A - Handwritten PDS (rev 2005)

 

•  B - NSO Birth Certificate (Original copy)

 

•  C - 2nd level eligibility (originally autheticated by PRC/CSC)

 

•  D - Diploma & Transcript of Records ( originally authenticated by School          Registrar)

 

•  E - Marriage Certificate ( if married,issued by NSO/Original copy)

 

•  F - Local Clearances (All original copies)

                    Police clearance

                    Municipal Trial Court Clearance

                    NBI Clearance

 

 *One (1) set of requirements in a LONG BROWN FOLDER, WITH TABBINGS, TABLE OF CONTENT, AND WITH FRONT LABEL CONTAINING APPLICANT'S FULL NAME, COMPLETE ADDRESS AND CONTACT NUMBER.
